WAIVER AND RELEASE OF LIABILITY, PARTICIPANTS ACKNOWLEDGMENT AND ASSUMPTION OF RISK AND FULL RELEASE FROM LIABILITY OF REALIGN FITNESS’S PROFESSIONAL SPORTS STRETCH: I acknowledge that the Professional Sports Stretch here-under includes participation in strenuous physical activates, including but not limited to sport stretch, calisthenics, weight training, various aerobic and various nutritional/fitness programs offered by Realign Fitness and I am assuming all risk associated with participating in this outdoors boot-camp program. By signing this agreement, participant asserts that he or she is capable of participating in the physical activities or sports stretch. Participant agrees to assume all risk and responsibility for not exceeding his or her own physical limits.Participant knowingly and voluntarily, on behalf of Participant and heirs, assigns and next of kin, agree to full release Realign Fitness (including owners, employees or other agents including Independent Contractors) from any and all liability, claims and/or litigation actions that Participant may have for injuries, disability or death or other damages of any kind, including but not limited to punitive damages, arising out of or relating in any way to participating in this Professional Sports Stretch.
